What is Community Inclusion?
Program leaders work with clients while they are in prison to develop person centred plans. These plans map out the help needed and steps the client must take to achieve their goals.
We can provide support for clients with:
- Employment and training
- Alcohol and drug support
- Mental and physical health
- Domestic assistance
- Personal support
- Family support
- Budgeting support
- Financial inclusion
- Social inclusion
- Recreational support
- Daily living skills
- Support with justice issues
- Support/Service coordination – episodic coordination
Who can participate in the Community Inclusion program?
The people we support are referred to us by Disability Services and Local Coordinators.
What agencies are involved with Community Inclusion?
Our Community Inclusion program is a collaboration between Outcare staff, guardians of participants, Disability Justice, Public Trustees, Community Corrective Officers, the Department of Disability Services, the Department of Corrective Services, and the Department of Child Protection. We work together to create a common language, assess behaviour management and risk management, and provide support while incarcerated and in the community post-release.
