What is Community Inclusion?
Community Inclusion is a NDIS-registered in-home and in the community support program for participants focusing on increasing their capability and independence.
We support participants to achieve their aspirations and build their capability. The program supports participants for the long-term, setting goals with them to increase inclusion within the community, connection with family and independence. We provide support with routine everyday tasks through to more complex tasks including building & maintaining relationships, social networks and attending justice appointments to assist understanding and compliance of bail conditions.
Through our person-centred approach, we ensure participant’s needs are met by managing our relationships with each participant and the people that are important to them. We demonstrate how participants can perform tasks to build capacity and independence rather than doing these tasks on behalf of someone.
Our staff walk alongside participants to help them achieve a range of essential tasks such as maintaining a home, applying and preparing for a job, and helping them build their financial literacy and relationships.

Who can participate?
Participants can be recommended to the program from their support coordinators, through other programs within Outcare or they can self-refer. Participants can be self-funded to join the program, they do not have to be funded through the NDIS and we can assist in their application for a NDIS plan to assess their eligibility.
